Why scaled Agile fails
Any one of the many scaled Agile anti-patterns can derail your delivery. In some cases, it requires a small intervention, in others, such as your high-level scope being unclear, you may have to do significant work to bring this back on track. A major issue in the industry is the focus on planning events as the definition of success. Teams create a backlog, but this simply isn’t enough. Here are some common anti-patterns:

Too much work in progress
Not using live data early when replacing a system
Fixed milestones rather than rough estimates
No system for removing blockers
Not structuring up front shaping
Not clear on high level scope
Not building in end to end slices and working out of sequence

Set up for success
There are no easy answers to scaling with large and complex work. Like following a recipe book, it is tempting to think that there is one perfect delivery method out there. In reality, there are good practices that you should follow to increase your chance of success. An example would be using Kanban to avoid pushing too much work into the teams. The other factor that is often ignored is ensuring you have the right people in place.
